Unwrapping the Level 4 Present will give you a random item from the game’s drop list, an 80% Battle Point Booster (500 minutes), The International 2013 Pass, and the Level 5 Present.

Receiving

You will automatically receive the Level 4 gift when you unbox the Level 3 gift.
